Problem sending a DMCA Notice, any help?

Hi guys, i want to send a DMCA notice to a site that lets people download some of my files for free. They have a form on their page to send DMCAs, everything was ok till they asked me for his:

””Upload pdf file that has approvement of Your rights for content” should be completed!”

Do you know how do i get that PDF or how do i do it? My english is not very good and i dont think i can send them a legal document wrote by me

I sent a complaint to Envato Support, they said they will do their best to handle it.

well, I only made a small modification to the template of fresh killer for these cases. Not accuse me, I just do it to make life difficult for a lot of jerks [those who want to give free my work]

There also seems to be quite a few sites popping up that have no way to contact them or DMCA ’s get completely ignored. Grrrrrrrrrrrrrrrrrrrr.
