Could you point me in the direction how to do a online design form that has the ability to upload your art and personalize with text?

This may be done using photoshop too, and I think that they will come out much better than the pre-made sites if you have to many pictures and don’t have the time I will gladly do it for you

No dude :)) you got me wrong. I said i can edit them for him 
sorry if that’s what came out but from what i can see he wants to add different images to a certain object. That takes time , and time is all I have
Sorry for the misunderstanding
Creating in flash what he asked for is kind of impossible, you have to make flash remove background from dynamic objects… know the borders, add them to the picture , skew and warp them… That’s highly unlikely to be possible, much faster to create a photoshop action that can do all of those things.

Anyway he wants something totally different from your description. He wants a simple “shirt designer”(i think this is the most used term for this on the web at the moment).. Just look at all the links that he gave: Do you think that bluecotton of cafepress were ‘impossible jobs’?
I have done similar projects in the past and i can tell you they are far from impossible
